Your next step will be to open your patient portal and complete your health history.  You will receive an email within the next 24 hours from our software vendor Modernizing Medicine.

Once you have opened your portal, please complete the following sections in the My Health section of your portal prior to your visit:

• Medications
• Allergies
• Past Medical History
• Skin Disease History
• Social History
