Steps to becoming a new patient at Cape Cod Dermatology, LLC
Step 1
Contact your PCP
Cape Cod Dermatology requires that ALL new patients visit or contact their primary care physician to request clinical documentation outlining the specific reason you need to see a specialist.
This may be in the form of a letter, prescription, order, or medical referral.
Ask your physician to describe the nature of the problem or condition in detail and relay any close family history of skin cancer.
This may be faxed to 508-771-4355.
Step 2
Fill out new patient information
Choose How...
Patient Portal - send an email with your full name and date of birth to [email protected] to request a link.
Fillable Form - Click Here to fill out and submit your new patient packet. (no printer or log in needed)
Paper - Call our New Patient Coordinator at 508-771-9779 ext. 2 to request a paper packet be mailed to you or stop by at 134 Ansel Hallet Road West Yarmouth to pick one up.
Step 3
Wait for our call !
Currently Dr. Elizabeth Tocci and our Physician Assistant Bri Schreiner are able to accomodate new patients. They practice at our West Yarmouth location only.
When we have received the documentation from your physician and your new patient forms are complete, our new patient coordinator will reach out to schedule your first appointment.
