Steps to becoming a new patient at Cape Cod Dermatology, LLC
Step 1
Get medical referral from PCP
Please visit or call your primary care provider and request a medical referral describing your problem or condition requiring dermatology. We ask that your physician determine whether your need is urgent, emergent or for routine care.
This may be faxed to 508-771-4355.
Due to the number of requests from new patients, we ask that you help us triage your request by visiting or calling your primary care provider FIRST.
Step 2
Fill out new patient intake packet

If you prefer to use our paper form please call our office at 508-771-9779 to request one be mailed or stop by at 134 Ansel Hallet Rd W Yarmouth to pick one up from our outside box.
Step 3
Call to Schedule your first appointment!
Please wait 7 days after completing your new patient intake before calling to schedule. This will allow us time to update your information.
Then, reach out to our new patient coordinator at 508-771-9779 ext. 2 to schedule your first appointment.
NOTE: All calls are currently being forwarded to our business mobile line as our new patient coordinator is working from home. You may notice a short connection delay.
