Steps to becoming a new patient at Cape Cod Dermatology, LLC

Contact US
Please request a medical referral from your Primary Care Physician detailing the nature of your problem or condition. This referral should be faxed to (508) 771-4355.
New Patients are fit in to the schedule based on medical necessity and severity of symptoms. For urgent issues please have your PCP contact us directly.

Get Registered
Choose How...Our team needs 48 hours before your scheduled appointment to prepare your chart.
Fillable Form click, fill out, and submit your new patient information. (no printer or log in needed)
Printable Form click, print, fill out, and mail in your new patient registration.
Paper call 508-771-9779 Ext.1 to request paper registration forms or stop by the office to pick up.

Prepare for your visit !
Contact your primary care provider to request an insurance authorization if you have an insurance plan that requires it.
