Your first visit to Cape Cod Dermatology, LLC involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. Make an Appointment

In order to provide quality, effective care, we utilize an appointment schedule. Due to the number of requests from new patients, we ask that you help us triage your request. 

Please visit or call your primary care provider and request a medical referral describing your problem or condition requiring dermatology. We ask that your physician determine whether your need is urgent, emergent or for routine care. Once we receive this request, we will contact you to schedule your new patient visit.

You can contact our office Monday through Friday from 9 -12 and 1:30 – 4:00 to check on the status of your request to become a new patient.


Patient Forms 

You have a few options to filling out patient forms. Please choose a way below.

You can complete your information using our online form or using your patient portal.  Using your Patient Portal will allow you to change or update your information and health history at your convenience.  You will also be able to send messages to us and see your tests and results. Ask a staff member to email you a link to get started.
